Is Sarcopenia a Red Flag for Cognitive Decline After Menopause?

A recent study published in Maturitas by Vallejo et al. (2025) brings a fresh perspective to this discussion. It suggests that sarcopenia, the loss of muscle mass and strength, may not only impact mobility and physical resilience but could also signal an increased risk of cognitive decline in postmenopausal women.
What Did the Study Find?
The research was part of a larger study conducted by the REDLINC network (Research of the Climacteric in Latin America) involving over 1,100 women, with a focused sub-analysis on 772 postmenopausal women who had experienced natural menopause. The researchers used a simple screening tool called the SARC-F, designed to assess risk of sarcopenia, and compared it to outcomes on the MoCA test (Montreal Cognitive Assessment), which screens for mild cognitive impairment (MCI).
The results were significant:
Women at risk of sarcopenia (SARC-F score ≥4) were 2.44 times more likely to have MCI.
The prevalence of MCI jumped from 12.9% in women with no sarcopenia risk to 35.3% in those at risk.
Those with cognitive impairment were more likely to have a higher BMI, be physically inactive, and report more severe menopausal symptoms.
Importantly, protective factors included:
Higher educational attainment
Prior use of menopausal hormone therapy (MHT)
Why Does Sarcopenia Matter?
We’ve long understood the role of muscle mass in maintaining balance, bone strength, and mobility as we age. But this study highlights that loss of muscle strength may also be a warning sign of declining brain health. The postmenopausal period is already a time of increased risk for metabolic changes, inflammation, and hormonal shifts, all of which may accelerate both sarcopenia and cognitive decline if left unaddressed.
These findings aren’t isolated. Other studies, such as those by Maniscalco et al. (2024) and Dong et al. (2024), have also identified links between sarcopenia and reduced executive function, memory, and verbal fluency. Taken together, these insights support a more integrated approach to preventive care for women after menopause.
